It depends on the policy. The Government of Canada advises getting written confirmation that a pre-existing condition is covered, with a stability clause that sets out the period before your trip during which your condition, symptoms and medications must not change.
Do I need separate travel insurance if my card includes it?

Not necessarily, but verify the coverage. The Government of Canada says to check the terms, conditions, limitations, exclusions and requirements before you leave, and to make sure the policy covers medical evacuation, pre-existing conditions and repatriation.
How do I make a claim?

Follow the claims section of your certificate, which names the claims administrator and sets deadlines. For medical claims abroad, the Government of Canada advises getting a detailed report and invoice before leaving the country where you were treated and submitting the original receipts.
Is credit card rental car coverage the same as the rental company's?

No. CIBC's certificate, for example, covers only theft, loss or damage to the rental vehicle, not third-party liability or personal injury, and requires you to decline the rental company's collision or loss damage waiver. Check liability coverage with your own auto insurer and the rental company.
Is balance insurance the same as the insurance included with my card?
